We have a staff of around 100 employees and operate between two large clinics, one in Suffolk County and one in Nassau County, both of which are clean, modern, and newly renovated in 2020 and 2023. Our main site, a 26,000 sq. ft. state of the art facility located in Commack, houses all of our non-patient-facing staff, including case management, member services, and the remainder of the administrative staff.

Nurse Practitioner - Commack / Westbury -
- Conduct monitoring examinations for the WTC Monitoring Program patients under the supervision of the Associate Medical Director or designee.
- Provide treatment to WTC Program patients for WTC covered conditions including but not limited to aero digestive diseases and psychological and social problems. Actively participate in our Collaborative Care Model of healthcare delivery.
- Make referrals as appropriate to other healthcare providers and/or consult with physicians for further definitive therapy.
- Perform necessary administrative duties associated with the WTC program including but not limited to completing final letters, Worker’s Compensation, SSI (supplemental security income), LODI (line of duty injury), attorney- requested forms, letters, as well as certification for WTC-related illnesses, which is required of member services.
- Work with Cerner electronic medical records system for all patients charting documentation.

Registered Nurse - Nursing Administrator - Commack / Westbury -
- Deliver comprehensive patient care and care coordination for individuals with complex and chronic health conditions, ensuring continuity and quality across clinical services.
- Educate patients on diagnoses, treatment plans, cancer screenings, and program benefits to promote adherence and informed decision-making.
- Perform clinical duties including phlebotomy, spirometry, injections, vital signs, and medical questionnaires and cognitive assessments.
- Serve as a primary point of contact for patient inquiries via phone and patient portal; triage clinical concerns and coordinate timely follow-up care.
- Conduct patient assessments to evaluate acuity, barriers to care, and eligibility for case management services.
- Manage and monitor patient cases, including hospitalizations, discharge planning, referrals, and pre-authorizations for tests and treatments.
- Collaborate with interdisciplinary teams - including providers, social workers, pharmacists, and case managers - to ensure coordinated, patient-centered care.
- Support patient adherence by identifying care gaps, conducting outreach, and facilitating appointments with internal and external providers.

Social Worker - Commack / Westbury -
- Provide clinical assessment by reviewing self-report measures or administering clinical psychiatric diagnostic interviews, and if warranted, refer to community counseling. Please note this part of the job will require face-to-face interaction with our members.
- Care/case management for World Trade Center Health Program (WTCHP) members with complex medical health conditions, mental health conditions, and/or cancer conditions.
- Educate patients about WTC Health Program benefits and limitations.
- Perform appropriate referral and disposition planning.
- Put together prior authorizations for services and authorize covered tests, procedures and treatments. Put together higher-level authorization request for internal providers review.
- Liaison between members, our providers, and external providers treating members.
- Provide supportive services to members and families.
- Assist with WTC Certification process and work closely with Certification team.
- Use clinical skills in supportive counseling for members dealing with mental health concerns. · Facilitates referrals to community resources.
